Clinic Covid-19 Screening

To ensure your safety and the safety of our community and staff, you will be asked a series of questions prior to attending your scheduled appointment. These will be sent via a SMS. We kindly ask that if you answer YES to any of the following questions that you call the clinic as soon as possible.

Please do not attend your appointment if you:

We request that you reschedule your appointment if you have been in contact with someone confirmed or suspected to have COVID-19 or have recently become unwell, including experiencing any flu-like symptoms

We apologise for any inconvenience that this may cause.

Keeping Us All Safe

The practice requests all patients when attending their appointment sign in via the ServiceWA App using the QR code or sign the manual register available on the reception desk. Please wear a mask and sanitise your hands with the hand gel provided.

Additional Safety Measures

Check Exposure Sites

If you or anyone in your household has visited an exposure site, please DO NOT ATTEND. Please call the team and we will reschedule your appointment.

Attend Alone*

Due to social distancing guidelines please limit the number of people that attend your appointment.

*Where possible, please attend alone, or a maximum of 1 parent/carer if needed. If more than 1 parent/carer is required, please call the practice before the appointment to inform us.

Remain In Your Car

Due to limited space in our waiting area, please remain in your car and inform reception that you are here, by calling the practice number. 

A member of our team will call you as soon as you are able to enter the premises for your appointment

Your Appointment

Upon arrival, you must observe the strict social distancing rules, sign in via the ServiceWA app or contact register, and we ask that you use the hand sanitiser, as soon as you enter the practice. 

Masks MUST be worn at all times by adults & children aged 12 and above as dental practices are classed as healthcare settings.
